It provides your Gokhale Method Teacher with important insights into your lifestyle, health, and pain history. Completing the form before your scheduled appointment is highly recommended to ensure your session is both productive and comprehensive.
How to Complete the Intake Form:
- Visit https://gokhalemethod.com and log in to your account.
- Go to your User Dashboard by clicking the Profile Icon in the top-right corner of the page.
- Select "My Data" and navigate to the "Intake Form".
- Complete all five sections (Parts 1–5) of the form.
- Be sure to click 'Save' at the top and bottom of each section. Click next if you need proceed to the next section.
